A narrow mandate: get your records. Nothing more.

To request records on your behalf, providers need to see that you’ve authorized us. That authorization is a limited healthcare power of attorney — deliberately scoped to record retrieval only.

RECORDS RETRIEVAL Requesting copies of your medical records from your providers and insurers

OUT OF SCOPE Making any medical decision or giving consent to treatment for you
OUT OF SCOPE Sharing your data with anyone without your explicit instruction
OUT OF SCOPE Anything beyond obtaining your own records

Guarantees

REVOCABLE ANYTIME Withdraw it whenever you want, with immediate effect.
SURVIVES CANCELLATION It’s yours — you can keep using it to gather records even if you leave Corpus.
JURISDICTION-AWARE In Germany this combines a Vollmacht with a Schweigepflichtentbindung; we use the right instrument for your region.
